Complete the application and necessary paperwork and mail to: Camp Lawroweld 91 Allen Avenue Portland, ME 04103. 2. Distribute the recommendation forms to three individuals, other than family members, such as teachers, deans, work supervisors, pastors. Ask them to mail the completed form to the address shown. Recommendation forms are only required for those applying for the first time, not needed for returning employees. 3. Have a physical exam and ask the doctor to complete the medical form. 4. Please supply us with a copy of the following: Driver's License, or Any pictured I.D. or Passport Social Security Card Nursing License (if applicable) 5. Parents please sign paperwork where required if the employee is a minor (under 18 years of age). 6.
