Distribute the recommendation forms to three individuals, other than family members, such as teachers, deans, work supervisors, pastors. Ask them to mail the completed form to the address shown. 3. Have a physical exam and ask the doctor to complete the medical form. 4. Please supply us with a copy of the following: Driver's License, or Any pictured I.D. or Passport Social Security Card Nursing License (if applicable) 5. Parents please sign paperwork where required if the employee is a minor (under 18 years of age). 6. It is your responsibility to see that all required forms are received by our office by March 1st, 2008.
